Forms

All forms needed for treatment with Dr. Patel will be shared with you through Luminello. These forms can be reviewed and electronically signed through the Documents tab in your Luminello patient portal.

Telemedicine/Teletherapy

Dr. Patel uses Doxy.me for her telehealth visits. Doxy.me is compatible with smartphones and computers.

Returning Patients

• Please use the Patient Portal link to login to Luminello. 
• On Luminello, you will find information regarding your upcoming appointments, billing information, documents, a link to your telehealth visit, and secure email for messaging Dr. Patel.
